On November 4, 2008, voters passed the Michigan Medical Marihuana Act, making a powerful medical alternative available to tens of thousands of waiting patients. To qualify for the medical marijuana program in Michigan, you must have a diagnosis of a debilitating condition, as defined by the legislation. Currently, patients who have been diagnosed with cancer, HIV/AIDS, Crohn’s disease, nail patella, amyotrophic lateral sclerosis (Lou Gehrig’s Disease), glaucoma, cachexia (or wasting syndrome), seizures—including characteristics of epilepsy, severe muscle spasms—including characteristics of multiple sclerosis, a condition that causes severe and chronic pain or severe nausea are eligible to register for the program. Request a copy of your medical records, and bring the information to your medical marijuana evaluation. You can find a physician who performs medical marijuana evaluations online, or talk to your current physician.
To register as a patient in the program, you must submit a completed patient application packet, application fee, and a clear copy of your photo identification at the time of application. If you present a form of identification that is not from the State of Michigan, you must also submit a copy of your Michigan voter registration card as proof of residency. In cases where the application is not completed by the individual to be named on the card, a copy of Medical Durable Power of Attorney must also be submitted. The patient application packet includes a form that certifies your condition and must be completed by your doctor. The patient application packet is available in printable form on the State of Michigan website. It is helpful to print more than one copy, as applications with white-out or other alterations will not be accepted.
